• All Registry Forms MUST be submitted with the Document Submission Log which provides a checklist of requirements needed to complete each form.
• All Registry Forms MUST be printed double sided. Users without "Duplex" printers should print out page 1, then re-insert the paper and print page 2 on the reverse side.
