Below is a list of all the official forms required for the various transactions conducted by the Lands & Survey Department. - All Registry Forms MUST be submitted with the Document Submission Log which provides a checklist of requirements needed to complete each form.
- All Registry Forms MUST be printed double sided. Users without "Duplex" printers should print out page 1, then re-insert the paper and print page 2 on the reverse side.
